About Your Visit
The goal of our practice is to provide patients with excellent medical care in a friendly, caring environment. To better assist you and ensure prompt payment by your insurance carrier, we will make a copy of your insurance card and driver’s license or photo identification card. Updates on patient information are required at every visit.
Same day scheduled and walk-in patients will see first available provider. Walk-in patients are worked into the daily patient schedule around scheduled appointments. The wait for a walk-in visit may be prolonged (up to 2 hours). Your appointment may need to be rescheduled if you are more than 15 minutes late.
School/Sport Forms
- Must have a current physical (within 12 months).
- $10.00 fee for provider’s signature on forms.
Vaccine Records
- One free copy will be provided at your well check visit.
- $5.00 fee for additional or faxed copies.
Medical Records
- $25.00 fee for entire chart, may be more for records more than 25 pages, in accordance with Texas State Board of Medical Examiners provisions.
- Allow 2 weeks for processing.
- Signature of medical release form required.
Prescriptions
- For refills, please leave a message for the medical assistant.
- Please allow 24 hours for processing.
- Office visit within a year required for prescription refills (less for some medications).
Miscellaneous
- $25.00 fee for Letters of Medical Necessity.
- $5.00 fee for Billing Records.
After Hours
- After hour calls will be answered by our Tele-Nurse service. Registered nurses trained to answer your medical concerns are available 7 days per week including weeknights, weekends, and holidays.

Billing & Insurance
- We accept most major insurance plans.
- Full payment is expected at the time of service unless prior financial arrangements have
- been made with our management staff.
- Any deductible, co-payment and/or co-insurance must be paid at the time of service.
- Past due balances are required to be paid prior to visit.
- Self-pay visits (without insurance coverage) are provided at a discounted rate. They must be paid in full at the time of service.
- We accept Visa, MasterCard, and cash payments.
